How We Help
HomeCare is our certified home health service. Our experienced nursing staff, home health aides and homemakers specialize in caring for people with mental health challenges or developmental disability, while providing hands-on care for any number of other health concerns.
All HomeCare services require a physician referral, and are provided as part of a plan of care coordinated with the patient’s physician.
HomeCare services are available in Franklin, Hampshire, and Hampden Counties, and in the North Quabbin area.
What We Offer
We help individuals and families understand, manage and improve their health. This may include teaching people to balance the effects of various medications, addressing lifestyle changes needed to control chronic health conditions, and treating specific health complications.
Depending on the needs of the individual, services may include:
- Home visits
- Teaching and treatment to promote health
- Referral and coordination with other community services
- Skilled assessment of individuals with mental illness who are at risk of other health problems
Nurses evaluate the health situation of each individual and deliver a broad spectrum of services, from routine dressing changes to specialized psychiatric care. Nursing aides assist individuals with personal care, exercise programs, and special diets. Homemakers assist people with household duties, including shopping, cooking, laundry, and housekeeping.
Services may be covered by private insurance, Medicare, MassHealth, Commonwealth Connector programs, or private pay.
