Innovative Care Partners is a unique partnership of three established community organizations
(CHD, ServiceNet, and Gandara Services) that offers behavioral health and social services to
people throughout western Massachusetts. At ICP, our job is to help you feel better and live
your best life.
Innovative Care Partners needs CARE COORDINATORS with sharp minds and big hearts to
join our growing team. Using your teaching/coaching skills and community health connections,
you’ll empower clients to achieve their best possible health outcomes by pulling together many
of the resources they need. Bachelor’s Degree or CADC required. Bilingual (Spanish/English)
capacity is a plus.
